I know this question has been asked a lot but I simply can't figure out what the problem is here. I am still a noob with PHP and I desperately need help please!
Problem:

This is quite simple. You can't have any html output to the browser before your header function. The error tells you exactly where the html output is located.
So you have to change your script in the way you're first doing your code and header, and after that you put your html code.
