楼上一大堆代码，以为是你贴的，看了半天才反应出来不对，你这个功能是图形界面的，要会使用swing或者awt
先画出来界面，然后通过代码写出这些文本，加一个按钮提交请求参数，在数据库处理就哦了，
由于图形这块我不是很会，也不是主流总用，所以不能给你具体写出代码，你可以去网上找一些例子参考！
